Welcome to the Nacogdoches County Clerk's vital records online ordering system. Online requests will be processed during normal business hours, 8:00 AM to 5:00 PM, Monday through Friday, excluding holidays.

Please note the following:

New baby birth certificates may take up to ten (10) days for registration and availability.

Certificates for recent deaths typically take up to eight (8) days or more from the date of death for registration and availability.

Please ensure that the shipping address is correct, the Vital Records Office is not responsible for items lost in the mail. 

Pick up orders must be picked up by the person completing the application.

This is an official request for a Vital Record from the Nacogdoches County Clerk's Office.  Vital records include birth (including home births), death, and marriages that occured within Nacogdoches County.  The information contained in this request should be considered private. Please complete all information in full and then finalize the order process through payment of the 'order fee'.

You will be notified by email when your document is mailed or ready for pick-up.

The fee payment will be paid through Permitium Payments.  The charge will show on your credit card statement as 'PermVitalRecs'. This is a secure and trusted provider of payment services.

Please note the following:

  • Birth records for Nacogdoches County and the State of Texas are available back to 1903.
  • Death Records are available only for decedents who pass away within the Nacogdoches County limits.
 

Attention: The maximum number of Birth Certificates that can be ordered per person, per lifetime is TEN (10).

Processing of Applications:

- You will receive an email notification when your application has successfully been submitted into the online ordering system.

- You will receive a second email notification and a phone call if our office has notified you of needed documents or questions regarding your order.

- Once your application has been Approved, a confirmation email will be sent to notify you that your document is ready for pick-up, or will be mailed the following business day. The document can ONLY be picked-up by the applicant that places the order.

- If your application has been Denied, you will be notified by email.  The service fee, and any related credit card fees are considered NON-REFUNDABLE.  The remaining Payment Amount (certificate fees, shipping fees) will be refunded back to the applicant's credit card or bank account. 

 

* Warning: The penalty for knowingly making a false statement in this form can be 2-10 years in prison and a fine up to $10,000 (Health and Safety Code 195.003)

The Texas Department of State Health Services requires that all applicants upload a form of identification when submitting their application. You will be asked to select the method of receiving the certificates during the application. You will be notified by us once we have processed your application and the certificate(s) are out for mail or ready for pick-up. If you select "pick-up", ONLY the person submitting the application can pick-up the certificate(s).
